What is the cost to add a name to the glass artwork?
The cost is $25 per name with a maximum limit of 25 characters (includes letters, spaces and punctuation).

Can I use payroll deduction?
Payroll deduction is not available for this opportunity.

When can I purchase a name for the glass artwork?
University Health staff members will have the first opportunity to be a part of this once-in-a-lifetime project beginning Friday, February 19 through May 1, or until we are sold out.

How do I purchase a name?

to purchase your name and provide all required information. Each entry costs $25. You can purchase up to four names using the website. If you would like to purchase more than four names, please contact the foundation at

It is important to verify the accuracy of how the name should appear before you complete your purchase. The information you provide will be exactly how the name is etched on the glass artwork.

What size will the names be? Can I make an additional donation for a larger size?
Each name etched will be approximately 1 inch tall and all names on the wall will be the same size to allow for as many as possible. Names cost $25 each.

Will I receive an acknowledgement to confirm my purchase?
Yes, a confirmation receipt will be sent to the email address you provide at time of purchase.

If I do not wish to use a credit card to purchase online, how can I participate?
You will find a downloadable form on the Foundation infoNET page to complete with payment delivered to one of the Cashier Windows. You may also mail the form and donation to University Health Foundation, P.O. Box 33038, San Antonio, TX 78265-3030. We are unable to reserve a spot and take any names over the phone.

If I purchase multiple names, will these be listed together in one spot on the wall or alphabetical?
The artist is tasked with the placement of the names based on character counts, wall dimensions, and artistic plan for the glass artwork so we are unable to accommodate this request.

Can I donate to the project without providing a name?
Yes, you will be able to make a donation on our website and indicate you do not wish to provide a name for the glass artwork.

Can I purchase a name in memory of or to honor someone?
Yes, that is a lovely way to remember someone special. Only the individual’s name will be listed without an indicator of in memory or in honor due to spacing constraints. Please note that we are unable to send an acknowledgement to the individual or family member.

Can those outside of University Health participate?
We aim to give our current staff, retirees and volunteers first chance to be a part of this unique opportunity. After March 1, the number of name spots left available will be opened up to the community. So don’t delay—there are only 20,000 spots available!

Does this count toward my contribution to the Women’s and Children’s Capital Campaign or our Employee Giving program?
No. This special opportunity is beyond regular contributions through Employee Giving or special gifts to a fund or campaign. It is a unique opportunity to have your name etched in the history of University Health!

Is there a deadline to purchase a name?
Yes, the last day to purchase a name online is Sunday, May 1 if we have not “sold out” prior to that date. If you are mailing in a form with your donation, this must be received by Apr 15. We cannot accept any new names or changes to names after that date.
