Annual Wellness Visits for Medicare Patients

A preventative service visit is covered by Medicare at no cost to you. Our clinical staff will help develop a personalized care plan to meet your needs. Your annual medical appointment may include a health risk assessment, health screenings, a mammogram or other health related exams.

Advance Care Planning Classes

Provides information on your medical power of attorney and advance directives. Sessions include free notary public services.

Plan ahead for unforeseen circumstances. You can avoid putting difficult medical decisions about your care on the shoulders of multiple family members - prevent conflicts. The Medical Power of Attorney and Advance Directives ensure that your wishes are followed and understood.

Medical power of attorney is a legal document that allows you to choose another person to make medical decisions when you are no longer able to make your own decisions or to communicate those decisions.

Advance directives are legal documents that allow you to specify what actions should and shouldn’t be taken by your medical team should you become unable to communicate healthcare decisions. Advance directives establish decisions for your doctors and caregivers so they can follow your guidelines in the event that you become terminally ill, seriously injured, progress into late stages of dementia or a similar condition.

For your convenience, these classes are offered in English and Spanish.

Ask the Expert Series and Community Events

This series of educational programs provide an opportunity to talk to medical experts about various topics of interest to our aging population. We cover the importance of nutrition, exercise, chronic diseases and more.

Fall Prevention and Awareness Program

This program offers classes on how to prevent falls, slips and trips in your home by creating a safer environment. We teach you how to develop and follow safety checks.

Access to Our Fitness Center

Access our exercise fitness gym for only $6 per month with your PlatinumU membership or attend our on-site exercise classes. If you’re not a PlatinumU member and you have a referral from a doctor (60 or older), you can still join our fitness center for only $12 per month. We also offer free, low-impact arthritis and aerobic exercise classes. Health staff is available to assist with exercise equipment and training.

Diabetes and Nutrition Education

This educational program provides opportunities to learn more about diabetes, disease management and cooking classes ($5 per class, free if you’re a PlatinumU member).

Diabetes is a serious disease and one that affects millions of Americans every year. The San Antonio population has a high rate of prediabetes and diabetes. We’re fortunate to have access to the Texas Diabetes Institute – America’s largest diabetes center dedicated to the prevention and treatment of this complicated disease.

Eating healthy, exercising and losing weight is a common goal among diabetics. Our goal at Senior Health Services is to promote and provide ongoing activities that help you become healthier. Diabetes is just one condition improved by a balanced diet and increased physical fitness.
