Admissions Office
When you first come into University Hospital, please go directly to the Admissions Office. It is located on the first floor in the main lobby. An admissions specialist will assist you with:
- The appropriate paper work, and help filling it out;
- Advice concerning any financial requirements
before you are admitted and
- Directions or an escort to the appropriate hospital department.
If you are being admitted through the Emergency Center or Clinic, the emergency admitting staff will coordinate your admission.
Personal Property
If needed, your property/valuables can be inventoried and itemized on Form 95 at the time you are admitted. Any valuable items that you then bring to the hospital will be stored in the Admissions Office when you in-process. The original copy of Form 95 will be placed in your chart. You will be given a copy of the form that you must keep in order to collect your valuables after discharge. Once your discharge date from the hospital is known, you will need to return to the Admissions Office to pick up your valuables. In addition to your copy of Form 95, you will also need to show your hospital ID or photo ID to pick up your valuables. Make arrangements to pick up your valuables between 7:30 a.m. and 5:30 p.m.
