When your anticipated discharge date is determined, it is important to prepare as far in advance as possible. Your medical team will assist you and your family in planning any home care or resource referrals you may need during your recovery.
Checklist
Before you leave the hospital:
- Pack your personal belongings;
- Reclaim valuables from the hospital safe in the Admissions Office;
- Reclaim any medications, vitamins or herbs you brought from home;
- Leave a forwarding address for mail;
- Make financial arrangements regarding payment of your bill;
- Check with your nurse concerning any special
instructions for your care at home and
- Confirm date, time and location of follow-up
appointments.
If you forget any clothing or other personal property, call the operator and ask for the nursing supervisor from your unit.
Medical Supplies and Instructions
Continued quality health care at home is vital to your long-term well-being. If you have specific discharge requests or needs, be sure to tell your nurse. Our hospital staff will help make any arrangements that may be necessary. They can provide you with:
- Necessary supplies and equipment.
- Verbal and written instructions concerning appointments, medications and prescriptions.
