Who is Eligible?
- All new CCRx members within 90 days of enrollment can participate
- Medication Therapy Management Session
CCRx determines eligibility based on patients' number of chronic conditions, number of prescription medications, and expected prescription drug costs.
A patient must:
- Have at least one of these chronic conditions: diabetes, high cholesterol, high blood pressure, asthma, chronic pain.
- Take 10 or more prescription drugs.
- Be likely to spend at least $4,000 per year on prescription medications.
