Health Careers Coordinator Checklist

The following eight (8) steps in the process for student placements must be completed no later than three (3) weeks prior to the beginning of each clinical experience. Placement will not begin until all of these steps are completed.

  1. Ensure that your educational institution has a current affiliation agreement with University Health System by viewing the Student Affiliation Agreement list.
  2. Complete an application for Internship at least four (4) weeks prior to the desired start date.
  3. An in-person interview will be required prior to acceptance.
  4. Immunization records must be up-to-date before placement in our facilities.
    1. Tuberculosis test
    2. Current immunizations for:
      1. Diphtheria-Tetanus
      2. Measles
      3. Rubella
      4. Mumps
      5. Hepatitis-B.
  5. Faculty and students are required to read the Guideposts For Faculty/Student Orientation before students begin in our facilities.
  6. The final step is to submit the Online Rotation Form with the names of all students. Print the confirmation page that follows the Online Rotation Form.
  7. Before students begin the internship / externship, provide the confirmation document to the University Health System supervisor where students will be placed.
  8. While in our facilities, students should always have a Skills Requirement Checklist in their possession.

NOTE:

  1. Due to limited space, students are not allowed to park in the University Hospital parking lot. Many students park at the Crossroads Mall Park and Ride.
  2. Students are required to wear their name badges at all times. Your school badge is sufficient.
