Listed below are the research forms and instructions necessary to apply for UHS research protocol. Contact the UHS Research Projects Coordinator 210.358.0086 if you have questions or need forms not provided here.

The forms names listed next to the following icons are available in MS Word , Adobe PDF , or MS Excel formats. Click on the icon for display or download to your PC. The word documents are formatted to allow movement between fields by using the tab key.

Recruitment Submissions will require all those listed below as well as a disclosure log that must be submitted electronically on a monthly basis. There must be approval from the UTHSCSA IRB for waivers. In lieu of the waivers, signed signed authorizations from each subject prior to looking at their Protected Health Information (PHI) can be obtained and placed in the medical record.
